After completing his residency at the University Of South Carolina School Of Medicine Dr. R. Phillip Baker was the founding and managing partner in Piedmont Physicians for Women an OB/GYN practice for 32 years.  He has extensive background in managed care and contracting while acting as the President of an IPA and Chairman of the Board of a regional PHO.  Initially serving as a part time Physician Advisor doing mainly second level reviews took on the role as a full time Physician Advisor and Medical Director for Case Management and CDI in 2014.  Responsibilities also include denials management for Self Regional Healthcare a regional hospital in Greenwood, SC.  Dr. Baker serves on several committees including Co-Chair of the Finance Committee for My Health First an upstate South Carolina ACO, Corporate Compliance Committee for My Health First, Governmental Affairs Committee ACPA as well as several committees at the South Carolina Hospital Association. He was also appointed to the inaugural class of “Hirsch’s Heroes” in 2015 for his work with the process to file complaints with CMS against Medicare Advantage Plans. He joined the Board of Directors for the American College of Physician Advisors in 2017.

Dr. Lisa Banker is a native of Northwest Indiana and a graduate of the University of Notre Dame. She received her medical degree from the Medical College of Ohio, followed by an Internal Medicine residency at Riverside Methodist Hospital in Columbus, Ohio.

 

Dr. Banker is a board-certified internist and practiced with a large multi-specialty practice in eastern North Carolina for 14 years. She then took on a hospitalist role and ultimately served as the medical director for the program.

Dr. Banker is also the president of Ideal Health Care, Inc. and provides consultation services for clinical documentation improvement and medical-legal advice/risk reduction within the medical liability arena. She is a certified coder and clinical documentation specialist. 

Since 2011, Dr. Banker has been a Physician Advisor with notably broad responsibilities in utilization review, denials management and prevention, clinical documentation, physician education/outreach, revenue cycle integrity and regulatory compliance. Dr. Banker served as the Chief Physician Advisor for CarolinaEast Health System for five years and then accepted a position as the Corporate Vice President and Chief Physician Advisor for Case Management at McLeod Health in South Carolina.  Most recently Dr. Banker has returned to New Bern, Carolina and serves as the Chief Medical Advisor for Value Analysis and Revenue Integrity for the CarolinaEast Health System.

 



Alvin Gore, MD                                                                        Email Dr. Gore


A Board-certified Family Physician with extensive experience in all aspects of Utilization Management, Dr. Alvin Gore has been performing the duties of a Physician Advisor since 2008. Having completed his training in Family Medicine at the Memorial Center in Johnstown, Pennsylvania, he has remained involved in medical education for a number of years as an Assistant Medical Director of Family Medicine Residency Program.

He has been active in training Family Medicine residents and medical students, and has held academic appointments with a number of medical schools.

Dr. Gore had significant experience with both inpatient and outpatient patient management, with a particular interest in Utilization Management aspects of hospital practices. Dr. Gore has added Physician Advisors' responsibilities to his academic position at the hospital, gradually expanding his range of duties. He has become a successful liaison between the CM Department and the medical staff.

Dr. Gore subsequently accepted a full-time position as Director of Utilization Management at St. Joseph's Health in California, where he is currently responsible for the UM performance at two hospital segments of the Health System's Northern California region. He is responsible for all aspects of the Physician Advisor role including: 2nd level reviews, denial management/ALJ hearings, clinical documentation improvement projects, physician education and outreach, various LOS and cost of care reduction projects, as well as development of the local integrated delivery system.

Ms. Elizabeth Lamkin is a national speaker and author on operations, quality, utilization, and billing compliance. Her most recent publication with HCPro is The Revenue Integrity Training Toolkit.

Ms. Lamkin has over 20 years of experience as a hospital CEO and has served in teaching hospitals, non-profit and investor owned sectors of healthcare where she developed forward thinking systems to improve quality and utilization.

Ms. Lamkin is CEO and Partner in PACE Healthcare Consulting, LLC offering strategy, operations, analytics and software solutions to hospitals, physicians and other sectors of healthcare.

And she holds a Master’s in Healthcare Administration from the University of South Carolina, Columbia and BA Cum Laude, Core in Business Administration, from the University of South Carolina.







Steven Meyerson, MD, CHCQM-PHYADV                     Email Dr. Meyerson

Dr. Steven Meyerson graduated from Cornell University Magna Cum Laude with a BA degree in Biological Sciences and obtained his medical degree from the Albert Einstein College of Medicine in New York City.  He did his medical internship and residency in internal medicine at the Montefiore Hospital and Medical Center, also in New York City.  He is board certified in internal medicine and geriatric medicine.  In 2015 he became certified in health care quality and management with a physician advisor sub specialty (CHCQM-PHYADV) by the American Board of Quality Assurance and Utilization Physicians (ABQAURP).

Dr. Meyerson practiced internal medicine and geriatrics in Miami until 2003, after which he was Section Chief for the Extended Care and Sub-Acute Units at the West Palm Beach VA Medical Center. In 2004 Dr. Meyerson returned to Miami and served as Medical Director of the nascent hospitalist program at Baptist Hospital of Miami and after that, for nearly four years was full time medical director and physician advisor for the Department of Care Management.

In 2010 Dr. Meyerson joined the Accretive Health Physician Advisory Services where he was senior vice president of the Regulations and Education Group. In that role, Dr. Meyerson developed a national reputation for his many presentations, articles, lectures and webinars on case management and Medicare regulatory compliance for physicians, case managers, and other hospital professionals. He developed and edited the popular PAS UpDate, a monthly hospital industry newsletter. He has been a frequent contributor to the Monitor Monday and Finally Friday web casts and has presented online many times for RAC University. He is known for his frequent contributions to the rac-relief Google group and the ACMA’s LearningLink.

Dr. Meyerson is a member of the American Case Managers Association (ACMA), where he serves on the Public Affairs Committee, the Case Managers Society of America, the American College of Physicians and the American College of Physician Advisors, where he is a member of both the Board of Directors and the Governmental Affairs Committee. He also serves as the ACPAdvisors.org blog editor.

Dr. Meyerson received the 2014 Distinguished Service Award from the American College of Physician Advisors. He is currently a consultant in case management and hospital Medicare compliance.



Ven Mothkur, MD, MSc                                           Email Dr. Mothkur


Dr. Ven Mothkur is the Corporate Physician Advisor at Franciscan Health, a 14-hospital health system based in Illinois and Indiana. In his role, Dr. Mothkur leads mid-revenue cycle strategy and clinical operations including the organizational care coordination, utilization management, clinical documentation integrity, correct coding, clinical denial management, and physician advisory programs.

 

Prior to joining Franciscan Health, Dr. Mothkur transitioned from Family Medicine into healthcare consulting with Wildcat Venture Management, RxGen, & R1.



Bernard Ravitz, MD, PAC-CDI, CHCQM-PHYADV           Email Dr. Ravitz

     
Dr. Bernard Ravitz is a native of Detroit, Michigan where he completed a Residency program in Internal Medicine at Mount Carmel Mercy Hospital in Detroit, Michigan. Dr. Ravitz pursued a fellowship in chemical dependency at The National Institute on Alcohol Abuse and Alcoholism in Bethesda, Maryland where he now resides.

Dr. Ravitz is Board Certified with the American Board of Addiction Medicine. He practiced emergency medicine for seventeen years and is Board Certified in Emergency Medicine.

Dr. Ravitz currently serves as the physician advisor for MedStar Good Samaritan Hospital, Baltimore Maryland. President of the Medical Executive Committee, Medical Director of the Observation Unit, as well as multiple hospital and corporate MedStar Health committees.

Additionally, he is a Certified Physician Advisor in Clinical Documentation Improvement.



Ronald J. Rejzer MD, CHCQM-PHYADV                          Email Dr. Rejzer


Dr. Ronald Rejzer is a native of Northwest Pennsylvania and received his medical degree in 1980 from the Hahnemann Medical University of Philadelphia, followed by a residency at the Medical College of Virginia/VCU in Family Medicine.

Dr. Rejzer practiced full-time Emergency Medicine in the Tidewater area of Virginia for over 18 years and was Board Certified in both Family Medicine and Emergency Medicine. For the past 15 years, he has held various Physician Executive roles including VPMA and Chief Medical Officer positions for several prominent healthcare organizations, as well as Administrative Director for a large Mid-Atlantic Hospitalist management company.

Additionally, Dr. Rejzer has served as a consultant for Hospitalist programs, Utilization Management development, and Physician Advisor programs. He has participated on several hospital boards and was elected President of a local medical society in Virginia.

Dr. Rejzer has participated in Physician Advisor roles both part-time and full-time for over 12 years. He is a Diplomat of the ABQAURP and currently serves full-time as the Chief Physician Advisor at Parkland Hospital in Dallas, Texas.

Dr. Rejzer is Past President of the American College of Physician Advisors.



Erica Remer, MD, FACEP, CCDS                                        Email Dr. Remer


Dr. Erica Remer, a native New Yorker, graduated from State University of New York at Buffalo School of Medicine in 1985, and did her residency at University of Illinois Affiliated Hospitals Emergency Medicine Residency Program from 1985-1988. During a fellowship in emergency medicine administration, she developed expertise in documentation and professional fee reimbursement.

 

After practicing emergency medicine for 25 years, Dr. Remer became the first physician advisor at University Hospitals Health System in Cleveland, OH. She served in the role of Clinical Documentation Integrity Advisor for 4 years, training 2700 providers in ICD-10. Along her career path, she got certifications in medical coding and as a clinical documentation specialist.

 

Dr. Remer transitioned to independent consulting in July 2016. Although she loves helping identify documentation opportunity, her passion is educating coders, CDI specialists, and healthcare providers. She has authored numerous articles on CDI, coding, and denials, and is the co-host of Talk Ten Tuesdays, a weekly national podcast. For fun, she participates in ICD-10 Coordination and Maintenance Committee meetings.

 

She and her radiologist husband, Erick, reside in Beachwood, Ohio and are empty-nesters since their two sons flew the coop in August 2017.

Dr. Howard Stein
is a graduate of Nova Southeastern College of Osteopathic Medicine and completed residency in Family Medicine at Union Hospital in New Jersey. He received his Master of Health Administration from the University of Maryland in 2014.

He currently serves as the Associate Director of Medical Affairs and Physician Advisor at Centrastate Medical Center in Freehold, New Jersey. 

He is a board-certified Family Physician who served as an Assistant Clinical Professor of Family Medicine at Robert Wood Johnson Medical School and at the University of Medicine and Dentistry. He is board-certified by the American Board of Quality Assurance & Utilization Review Physicians.
 

He also served as a physician in the United States Air Force. He is married for 29 years with five children.



Juliet B. Ugarte Hopkins, MD, CHCQM-PHYADV  Email Dr. Ugarte Hopkins


Dr. Juliet Ugarte Hopkins is a graduate of the University of Illinois College of Medicine and served as co-chief resident while in her Pediatrics residency at Advocate Lutheran General Children’s Hospital in Park Ridge, Illinois. Dr. Ugarte Hopkins practiced as a pediatric hospitalist for a decade and also served as medical director of pediatric hospital medicine and vice-chair of pediatrics at Rockford Health System in Rockford, Illinois before transitioning into her current role as the first Physician Advisor for Case Management, Utilization, and Clinical Documentation at ProHealth Care, Inc., a two-hospital, 575-bed health system in Southeastern Wisconsin.

Dr. Ugarte Hopkins is board-certified in health care quality and management with a physician advisor sub-specialty by the American Board of Quality Assurance and Utilization Physicians (ABQAURP). She is the first physician board member for the Wisconsin chapter of the American Case Management Association (ACMA), a member of the RACMonitor editorial board, and she was recognized as a “Hirsch’s Hero” by Dr. Ronald Hirsch in 2016.

She lives with her husband in Northern Illinois where they valiantly strive to raise three daughters and a son into strong, thoughtful, and self-assured individuals with a corny sense of humor.
